Serbian Dossier Forms

Serbian International Adoption

Steps Overview – Traditional

Fill out the forms

More
Fill out the forms above.

Note: Bulgarian Medical Checklist, is something that the Bulgarian commission uses to help match you with a child based on the medical needs you are willing to consider.  Our social worker is a resource, you can talk to her as you fill it out, as well as your local social worker (if you live in a different state than NC).

Note:  For the first 6 questions on the Medical form, we ask that you consider saying “yes” because in practice this deals with the medical history of the bio parents.  In some cases, they may not even know who the bio father is, and info about the mom may be missing.  Saying ‘yes’ therefore to those first 6 questions, we believe, places you in a pool that can match you to more children.  If you say no, and they don’t have the info on the bio parents, they may pass on your application, even if the child isn’t struggling with any of those conditions him/herself.
