International AdoptionHow To Fill Out I-800 Form and Its Attachements?
With our help, you will complete I-800 and provide documents from Article 16 of the Hague Convention. Doing so will result in CIS issuing your Provisional Approval, which will result in the issuance of t Article 5, by US Embassy in Sofia. Our legal team will use these documents to further your case in Bulgaria through the courts.

Part 2: Documents Provided by Agency:
Fear not! We have sent you all of these documents (bullet points 1 – 4 below). You don’t have to produce them yourselves.
You simply have to take what Lina mailed you (physically), take it out of the package and include it to the forms above! Easy!
The report which includes the information in Article 16 specified in 8 CFR Part 204.313(d)(3) and (4) as follow:
- A copy of the child’s Identification Picture and Identification Number through the Bulgarian Central Authority, on Bulgarian and English (for each child)
- A document, Certificate for Agreement With Proceeding With the Adoption Procedure, the Bulgarian Central Authority, The Ministry of Justice of the Republic of Bulgaria, which confirms that the parental rights have been terminated and also confirms its approval to the petition of the American citizen(s), to adopt the child(ren).
- A statement signed under penalty of perjury US law by Saint Mary’s certifying that the report as set forth in Article 16 is accurate, true, correct and complete copy of the report obtained from the Bulgarian Central Authority; and that all pre placement preparation and training has been completed; and that Saint Mary will ensure the post adoption supervision; and that the Bulgarian Central Authority have approved the prospective adoptive parents for adoption of these particular child(ren).
- The child’s background information – the medical and social information document on Bulgarian and English (for each child)

Conclusion
These are the instructions for how to complete your I-800 Form. In addition to it, you also need to complete: I864W, Supplement 1, Form G1145 and Exhibit C, Acceptance of Referral. This is all listed under “Part 1” of your guidelines above. Documents from “Part 2” are also required, but you don’t have to produce those. They have been mailed to you. However, you need to include them with everything else when you submit your entire I800 to USCIS.
